Honoring Choices Pacific Northwest invites you to join us at our Share the Experience on Wednesday, February 22, 2017!

We’re excited to host this forum where health care professionals can convene and have an open dialogue about implementing advance care planning programs throughout the region.

The keynote address will be delivered by Jennifer Brokaw, MD, emergency physician, co-founder of C2it Health, and daughter of Tom Brokaw. Check out the TedTalk she did with her father on the importance of advance care planning!
