At Evergreen, we believe all patients have the right to let their voice be heard when it comes to their medical care. With the help of Honoring Choices Pacific Northwest, we are here to help you walk through an advance directive in a workshop format. We will focus on experiences, “living well”, choosing a healthcare agent and what medical decisions would need to be made in the event of a serious sudden illness or accident. You will have the opportunity to share and hear from others as well as ask questions as we work our way through the advance care planning document.
